Parental Consent Form
Every Clubber who participates in Bible quizzing must turn in this parental consent form on the day of the meet.  Coaches, please download this form and have the parents sign them.

Here are the current guidelines for churches who want to participate in Bible quizzing!
Bible Quiz Roster Sheets
Fill these roster sheets out and bring them with you the day of the meet.

You may either register online or you can fill out this form and mail it in.
Bible Quiz Score Sheets
Here is a copy of the score sheets that we use.

Parental Consent Forms and One Day Worker Forms should be completed prior to arriving at event.
All Bible Quiz events begin at 1pm.
